Medical

Anthem Value HMO 20/40/20%

The HMO Plan offers comprehensive coverage at a low out-of-pocket cost to you and your family. The primary objective of an HMO is to have only one physician coordinate a patient’s care through the HMO network of physicians, medical offices and hospitals. Under this plan, you MUST choose a Primary Care Physician (PCP) and a medical group from the Anthem network. If you do not, Anthem will assign a PCP and medical group to you.

Anthem Classic HMO 20/40/500 admit/250 OP

The HMO Plan offers comprehensive coverage at a low out-of-pocket cost to you and your family. The primary objective of an HMO is to have only one physician coordinate a patient’s care through the HMO network of physicians, medical offices and hospitals. Under this plan, you MUST choose a Primary Care Physician (PCP) and a medical group from the Anthem network. If you do not, Anthem will assign a PCP and medical group to you. This plan is similar to the HMO 30/40/500, except you will trade reduced benefits for reduced price.

Prudent Buyer Anthem Exclusive Value PPO 30/30

The exclusive provider organization, or EPO, is a health insurance plan that only allows you to get health care services from doctors, hospitals, and other care providers who are within a certain network. Your insurance will not cover any costs you get from going to someone outside of that network. The only exception is that emergency care is usually covered. 

Prudent Buyer Anthem Classic PPO 1000

The PPO plan also offers you the freedom to go in and out of the network for care. You may use a PPO provider whose negotiated rates provide richer levels of benefits, with claim forms filed by the provider. You may also obtain services outside the PPO network; however, you will incur higher out of pocket expenses and you are responsible for filing claims.

Prudent Buyer Anthem Classic PPO 250/20/40/20

The PPO plan offers you the freedom to go in and out of the network for care. You may use a PPO provider whose negotiated rates provide richer levels of benefits, with claim forms filed by the provider. You may also obtain services outside the PPO network; however, you will incur higher out of pocket expenses and you are responsible for filing claims. This plan is similar to the Prudent Buyer Anthem Classic PPO 1000, except you will trade reduced benefits for reduced price.

Dental

Anthem DHMO

The DHMO provides dental care through a network of dentists. There are no deductibles, plan maximums, or claim forms. You must select a Primary Care Dentist to receive covered dental care or a referral to a specialist. Services are covered on a co-payment basis.

Anthem DPPO

The DPPO covers a wide range of dental services. You have the freedom to go in and out of network for care. You may visit a DPPO Dentist and benefit from the reduced contracted fee resulting in less out of pocket expense. You may also visit an out of network Dentist, where services will be based on a reasonable and customary amount and you will be responsible for any amount owed after insurance has paid.

The vision plan provides professional vision care and high quality lenses and frames through a broad network of optical specialists.

Life and AD&D

Mutual of Omaha Basic Life and AD&D

Life insurance is an affordable way to help you provide financial security for your loved ones. We provide coverage that is both comprehensive and affordable for you and your family. This benefit is 100% company paid.

Mutual of Omaha Voluntary Life and AD&D

Voluntary Life/AD&D Insurance provides additional coverage for Employees who want to supplement their group life and accidental death and dismemberment (AD&D) insurance benefits through Mutual of Omaha. When you enroll yourself or you and dependents in this benefit, you pay the full cost through payroll deductions.

Disability

Mutual of Omaha Voluntary Short Term Disability

The Voluntary STD plan provides you with disability income benefits for disabilities resulting from a non-occupational injury or illness.

Mutual of Omaha Long Term Disability

The LTD plan is designed to provide you with a reasonable level of income replacement in case you can no longer work due to a disability.

EAP

Mutual of Omaha EAP

We provide ALL employees and their families with an Employee Assistance Program (EAP) through Mutual of Omaha. The EAP offers confidential, professional counseling assistance and a 24-hour hotline seven days a week and provides support for a wide variety of personal, family and financial issues.

FSA

Navia Health Care FSA

We use Navia as our Flexible Spending Account (FSA) partner. If you elect to enroll, you can set aside pre-tax dollars to pay for eligible health, dental and vision care expenses.

Navia Dependent Care FSA

We use Navia as our Dependent Care Flexible Spending Account (FSA) partner. If you elect to enroll in the dependent care FSA, you may set aside up to $5,000 pre-tax per year for your dependents’ child care expenses.

Voluntary Worksite Plans

Accident

You can purchase Accident coverage for you and your dependents for a monthly premium with pre-tax payroll deductions. Accident insurance helps cover everyday expenses such as rent, mortgage, groceries and transportation while you recover from an accident. The plan covers from the initial emergency room treatment, hospital confinement, to follow up visits.

Hospital Indemnity

You can purchase Hospital Indemnity coverage for you and your dependents for a monthly premium with pre-tax payroll deductions. Hospital Indemnity helps cover expenses for things like emergency room and doctor’s visits as well as transportation and ambulance costs. Benefits are predetermined and are paid regardless of any other insurance policies you may have. Security normal retirement age.

Critical Illness

You can purchase Critical Illness coverage for you and your dependents for a monthly premium with pre-tax payroll deductions. The Critical Illness plan helps with the treatment costs of covered critical illnesses, such as a heart attack or stroke that you medical plan may not cover. More importantly, the plan helps you focus on recuperation instead of the distraction of out-of-pocket costs. With the Critical Illness plan, you receive cash benefits directly (unless otherwise assigned)—giving you the flexibility to help pay bills related to treatment or to help with everyday living expenses.

Transparency in Coverage

The Transparency in Coverage Final Rules require certain group health plans to disclose on a public website information regarding in-network allowed amounts and billed charges for covered items and services in two separate machine-readable files (MRFs).
